What Is PT-141?

PT-141, chemically known as Bremelanotide, is a cyclic heptapeptide and synthetic analogue of alpha-melanocyte-stimulating hormone (α-MSH). It was developed by Palatin Technologies and is notable for being an FDA-approved pharmaceutical (brand name: Vyleesi) for hypoactive sexual desire disorder (HSDD) in premenopausal women, making it one of the few peptides with full regulatory approval.

Mechanism of Action

Unlike most compounds studied for sexual dysfunction that operate peripherally (via vascular mechanisms), PT-141 acts centrally through the melanocortin receptor system — specifically MC3R and MC4R receptors located in the brain. This central mechanism is what distinguishes it pharmacologically from PDE5 inhibitors (such as sildenafil), which work peripherally through vascular smooth muscle.

MC4R activation in the hypothalamus and limbic system modulates neural circuits associated with motivation and sexual arousal, independent of vascular effects.

Clinical Development

The clinical development path for PT-141 was unusual — it emerged from research on melanotan II (MTII), a non-selective melanocortin agonist that showed unexpected pro-erectile effects in male human subjects during sunless tanning trials. PT-141 was engineered to preserve these central effects while reducing unwanted melanotropic (skin-darkening) activity.

FDA Approval

Vyleesi (bremelanotide) received FDA approval in June 2019 for HSDD in premenopausal women. It is administered as a subcutaneous injection approximately 45 minutes before anticipated sexual activity. The approval required evidence of increased sexual desire and decreased associated distress.
