Soft-Tissue Injury Recovery Stack

BPC-157 + TB-500 — the classic complementary stack for tendon, ligament, and muscle recovery.

Injury recovery & tissue repair Beginner Cycle: 4–8 week cycle, 2–4 weeks off

Overview

BPC-157 drives localised angiogenesis, fibroblast proliferation, and growth factor expression near the injection site. TB-500 (thymosin beta-4) acts systemically on actin remodelling and cell migration. The two have non-overlapping mechanisms, which is why they are a common complementary stack in the recovery literature.

Dose schedule

Phase Dose & route Notes
Loading — weeks 1–4 BPC-157: 250–500 mcg SC daily near injury site. TB-500: 2.5 mg SC twice weekly (total 5 mg/wk). Loading phase. BPC-157 subQ around the injury maximises local tissue concentration.
Maintenance — weeks 5–8 BPC-157: 250 mcg SC daily. TB-500: 2 mg SC weekly. Lower-intensity continuation. Many protocols drop BPC-157 to EOD at this point.
Off cycle — weeks 9–12 No peptides. Tissue consolidation. Receptor sensitivity recovers.
Optional second cycle Repeat loading if symptoms persist. Not usually needed. Stop if two cycles do not resolve the injury — reassess diagnosis.

Bloodwork cadence

Timing Panel
Baseline CBC, CMP, lipid panel. ESR/CRP if inflammatory injury suspected.
Week 8 Repeat CBC + CMP if cycling at higher doses.
As needed Imaging (MRI/US) to track structural recovery for high-grade injuries.

Expected timeline

Week 1
Reduced pain response often reported in 5–7 days (anecdotal).
Week 2
Improved range of motion in mild soft-tissue injuries.
Week 4
End of loading phase. Peak subjective recovery.
Week 8
End of maintenance. Most case reports taper here.

Red flags — stop and reassess

  • Any history of malignancy — TB-500 is angiogenic and should not be used in active or recent cancer.
  • Progressive pain or new swelling despite protocol — stop and get imaged.
  • Unexplained bleeding or bruising (rare, theoretical with TB-500).

Contraindications

  • Active cancer or recent malignancy (TB-500 contraindicated).
  • Concurrent anticoagulant therapy for TB-500 — low-risk but discuss with prescriber.
  • Pregnancy or breastfeeding.
Educational use only. This protocol summarises published trial schedules, FDA labelling, and widely-referenced research-community practice. Doses, cycle lengths, and bloodwork recommendations should be reviewed with a qualified prescriber before use in any human. Several peptides discussed are not approved for human administration in most jurisdictions.
